Assumption of risk, release of liability & indemnity
In this agreement, “The Route” means The Route and its Receiver Takeover program, together with their owners, officers, coaches, trainers, employees, contractors, volunteers, program partners, and the owners and operators of any facility where training takes place (collectively, the “Released Parties”). “Athlete” means the participant named above. “I” means the person signing below — the Athlete if 18 or older, otherwise the Athlete's parent or legal guardian.
1. Assumption of risk. I understand that football training is a physically demanding, contact-adjacent athletic activity, and that participation carries inherent risks that cannot be eliminated regardless of the care taken. Those risks include, without limitation:
- Sprains, strains, fractures, dislocations, and muscle, ligament, or tendon injuries
- Collisions with other participants, equipment, goalposts, or fixed objects
- Head, neck, and spinal injuries, including concussion and traumatic brain injury
- Heat illness, dehydration, cardiac events, and other medical emergencies
- Field, turf, and weather conditions, and travel to and from training sites
- Permanent disability, paralysis, and death
I knowingly and voluntarily accept and assume all such risks, both known and unknown, and accept full responsibility for the Athlete's participation.
2. Fitness to participate. I represent that the Athlete is in good physical health and has no medical condition that would make participation unsafe, and that any condition that could affect participation has been disclosed above. I agree the Athlete will follow all coaching instructions, safety rules, and facility rules, and will stop and report any injury, pain, or illness immediately.
3. Release and waiver of claims. To the fullest extent permitted by law, I release, waive, and discharge the Released Parties from any and all claims, demands, damages, causes of action, costs, and expenses of any kind arising out of or related to the Athlete's participation in any Receiver Takeover or The Route training, including claims alleging ordinary negligence by the Released Parties. This release does not apply to gross negligence, willful misconduct, or any liability that cannot be released under applicable law.
4. Indemnification. I agree to indemnify and hold the Released Parties harmless from any claim, loss, or liability — including reasonable attorneys' fees — brought by or on behalf of the Athlete, or arising from the Athlete's conduct, in connection with participation in the program.
5. Medical authorization. In the event of injury or illness and if I cannot be reached in time, I authorize The Route's coaches and staff to administer or arrange basic first aid and to summon emergency medical services, and I authorize treating medical personnel to provide emergency care, transport, and treatment for the Athlete. I accept financial responsibility for any medical care, transport, or treatment the Athlete receives.
6. Code of conduct. The Athlete is expected to show up on time, train with effort, and treat coaches, teammates, officials, and facilities with respect. The Route may remove an athlete from a session or the program for conduct that is unsafe, disruptive, or disrespectful, or for violating facility rules.
7. Payment, cancellations & missed sessions. Program fees are due before the first session. Season package sessions are reserved for the athlete for the full program dates and are not transferable to another program or season. Sessions may be rescheduled or relocated for weather, field availability, or safety; The Route will communicate changes as early as possible. 8. Photo & media. Training sessions are sometimes photographed or filmed for coaching review and promotional use. Your consent choice below governs whether the Athlete's likeness may be used publicly, and can be changed at any time by emailing us.
9. Governing law & severability. This agreement is governed by the laws of the State of Georgia, without regard to conflict-of-law rules. If any provision is found unenforceable, the remaining provisions stay in full force and effect. This is the entire agreement between the parties on this subject and supersedes any prior understanding.
10. Electronic signature. I understand that typing my name below and submitting this form creates a legally binding electronic signature with the same force and effect as a handwritten signature, and that a copy of this submission is retained by The Route as a record.
